    Intelligent Algorithm for Efficient Use of Energy Using Tackling the Load Uncertainty Method in Smart Grid

    In this paper, i am developing a unique optimization based real time inland load management algorithm that takes into account load ambiguity in order to minimize the energy payment for each residential user, as well as reduce the peak to average ratio to overcome the drawbacks in the stability of electrical grid. By categorizing the all residential load in different classes, i.e. must run, interruptible and uninterruptible appliances, i used the real time pricing scheme for load management. However, real time pricing creates the peak profiles when the energy demand is too high, that’s why i used the combination of real time pricing and inclining blocks rates model to improve the grid stability by reducing the peak to average ratio. A simulation results show that the proposed algorithm efficiently and effectively reduced the overall residential energy cost as well as peak to average ratio of our model for data provided

    Clinical, pathological and molecular factors predicting Axillary Node involvement in primary Breast Cancer in Pakistani women

    Background: Axillary lymph node involvement in primary breast cancer is one of its most important prognostic features. Thus any factors that may predict axillary lymph node involvement in this setting could be potentially helpful in treatment planning and other interventions. Objective: The objective of this study was to evaluate clinical, pathological and immuno-histochemical markers in univariate and multivariate analysis, which may be helpful predictors of axillary lymph node involvement in breast cancer. Method: A retrospective analysis of 555 cases. Of these 58% had axillary nodal positivity and 42% were negative. Conclusion: Factors of no significance included patient’s age, height, weight, age of first pregnancy, parity, marital status, menopausal status, family history of breast cancer, side of tumor. In univariate analysis the age of menarche, duration of symptoms, tumor size, site in outer quadrant, S phase and skin and nipple involvement all predicted axillary nodal involvement. The length of breast-feeding, increased intraductal component and increased PCNA were inversely proportional to nodal involvement. In multiple regression analysis however only size of the tumor, involvement of the skin and nipple and disease in the outer quadrant of breast were the factors, which assumed significanc

    Protective Effects of Salivary Factors in Dental Caries in Diabetic Patients of Pakistan

    Salivary factors have been studied for their effects on the process of dental caries in patients of diabetes mellitus type 2. In this study, protective role of salivary pH, salivary flow rate, and salivary calcium is assessed in the patients of diabetes mellitus type 2 with dental caries. The samples of saliva were collected from 400 patients of diabetes mellitus type 2 and 300 age- and sex- matched controls after getting informed consent. All the subjects were classified into four groups according to age. The severity of dental caries was counted by decayed, missed, and filled teeth (DMFT) score. The salivary pH, flow rate, and calcium levels were found to be low in patients as compared to controls. The levels of fasting blood sugar, HbA1c, and DMFT score were found to be significantly high in patients than controls. The glycemic factors were significantly correlated with salivary factors indicating their influence on progression of caries in diabetes. On the basis of findings, it is concluded that the suitable salivary pH and flow rate may be regarded as main protective factors against dental caries in diabetes. Optimum level of salivary calcium may be responsible for continuous supply of calcium to arrest the demineralization and help reduce the occurrence of dental caries

    Epidermal Growth Factor Receptor (EGFR) as a Prognostic Marker: an Immunohistochemical Study on 315 Consecutive Breast Carcinoma Patients

    Objective: To assess the independent and interdependent prognostic value of epidermal growth factor receptor (EGFR) in carcinoma of breast in female population. The Type I family of growth factor receptors includes epidermal growth factor receptor (EGFR also known as EGFRI). Methods: The expression of EGFR protein was analysed immunohistochemically on 315 tumour specimens of infiltrating ductal carcinoma of breast. These patients also had axillary lymph nodes sampling. Results: Overexpression and/or amplification of EGFR was observed in 70 (22.00%) tumours. Eleven (16%) were grade I, 43 (61%) grade II and 16 (23%) grade Ill tumours. Axillary lymph node metastasis had significant correlation with intensified positivity of EGFR (p\u3c 0.05). Significant number of EGFR positive patients developed local recurrence and distant metastases to brain, liver and bone (p\u3c 0.05). EGFR positivity showed significant correlation with the disease free and overall survival (p\u3c 0.05). At a median follow-up of48 (4 years) months in EGFR positive patients, the overall survival was 3.39 years and disease free survival was 2.86 years. EGFR negative tumour patients showed a better survival. In this group the overall survival was 4.62 years and the disease free survival was 4 years. Conclusion: EGFR analysis can be a useful indicator for the selection of patients who are at the high risk, for hormonal therapy decisions and can be useful as a target for new treatment modalities

    Fungal infections in patients with chronic liver disease: mortality and associated risk factors

    Background: Patients with chronic liver disease are immunocompromised and prone to different opportunistic infections. Fungal infections in patients admitted with liver cirrhosis are not rare and they may increase mortality and morbidity of these patients. Aims of the study is to determine the mortality and its risk factors associated with fungal infections in patients with chronic liver disease.Methods: In this retrospective study, patients admitted with chronic liver disease during the last four years on this hospital were studied for diagnosed fungal infections. A matched control group of cirrhosis patients with a ratio of 1:2 admitted without fungal infections was also studied and mortality was compared between the two groups.Results: Seventy admitted patients of liver cirrhosis with microbial and histopathological evidence of fungal infection were found while 140 patients of the control group had no evidence of fungal infection. Hepatitis C virus infection was the major cause of cirrhosis (65%) and most of the patients were in child class C(63%). Urinary tract infection, esophageal candidiasis, and mucormycosis were major fungal infections. Mortality was much higher in the fungal infections group (34.3%) as compared to the non-infectious group (16%). On multivariate analysis, high WBCs count, hypo-albuminemia and high creatinine levels were the worst factors affecting mortality.Conclusions: Fungal infections are a significant cause of morbidity and mortality in patients with decompensated cirrhosis. Advanced cirrhosis, renal insufficiency, and leucocytosis are independent predictors of fatal outcome in these patients

    Hormonal priming alleviates salt stress in hot Pepper (Capsicum annuum L.)

    Germination and seedling establishment are critical stages in the life cycle of plants especially under stress conditions. Different methodologies have been adopted by plant physiologists in different crops to alleviate salt stress. Seed priming has proven beneficial in this regard in many important agricultural crops. The effect of seed priming with salicylic acid (SA) and acetylsalicylic acid (ASA) in improving seed vigour and salt tolerance of hot pepper seedlings was evaluated. Concentrations over 1.0 mM of ASA or SA showed adverse effects on seed emergence. Seeds primed with SA (0.8 mM) and ASA (0.2 mM) were sown in medium at different salinity levels [0, 3, 6 and 9 dS m-1]. Both, SA and ASA treatments showed significantly better results over the control by improvement in time taken to 50% emergence, final emergence percentage, root and shoot length, seedling fresh and dry weight and seedling vigour. Overall, acetylsalicylic acid exhibited superiority over salicylic acid. Our results indicate that hormonal priming, especially with acetylsalicylic acid, can be a good treatment for hot pepper to enhance uniformity of emergence and seedling establishment under normal as well as saline conditions

    Transitional cell carcinomas of the urinary bladder. A histopathological study

    Objective: To determine the histological grading and muscle invasion in Transitional Cell Carcinomas of the Urinary Bladder; and to evaluate whether any correlation exists between tumour grade and muscle invasion. Method: A Retrospective study of all consecutive cases of Transitional Cell Carcinomas of the Urinary Bladder diagnosed at Aga Khan University Hospital between 1st Jan 1997 and 31st Dec 2000. Results: This study included 495 cases of Transitional Cell Carcinoma of the Urinary Bladder. M: F ratio was 4:1. Ages of patients ranged from 30 years to 87 years. Mean age in males was 59.1 years and in females 58.8 years. Forty four percent cases were Grade II, and 29.5% Grade Ill. None of the Grade I lesions were invasive, 10% Grade II tumours, 61 .41% of Grade Ill and all Grade IV tumours were invasive. Conclusion: There is a definite correlation between advancing tumour grade and muscle invasion

    Performance assessment of general circulation model in simulating daily precipitation and temperature using multiple gridded datasets

    The performance of general circulation models (GCMs) in a region are generally assessed according to their capability to simulate historical temperature and precipitation of the region. The performance of 31 GCMs of the Coupled Model Intercomparison Project Phase 5 (CMIP5) is evaluated in this study to identify a suitable ensemble for daily maximum, minimum temperature and precipitation for Pakistan using multiple sets of gridded data, namely: Asian Precipitation- Highly-Resolved Observational Data Integration Towards Evaluation (APHRODITE), Berkeley Earth Surface Temperature (BEST), Princeton Global Meteorological Forcing (PGF) and Climate Prediction Centre (CPC) data. An entropy-based robust feature selection approach known as symmetrical uncertainty (SU) is used for the ranking of GCM. It is known from the results of this study that the spatial distribution of best-ranked GCMs varies for different sets of gridded data. The performance of GCMs is also found to vary for both temperatures and precipitation. The Commonwealth Scientific and Industrial Research Organization, Australia (CSIRO)-Mk3-6-0 and Max Planck Institute (MPI)-ESM-LR perform well for temperature while EC-Earth and MIROC5 perform well for precipitation. A trade-off is formulated to select the common GCMs for different climatic variables and gridded data sets, which identify six GCMs, namely: ACCESS1-3, CESM1-BGC, CMCC-CM, HadGEM2-CC, HadGEM2-ES and MIROC5 for the reliable projection of temperature and precipitation of Pakistan

    Orientation Independent Chipless RFID Tag Using Novel Trefoil Resonators

    In this paper, a compact and fully passive bit encoding circuit, capable of operating as a chipless radio frequency identification (RFID) tag is presented. The structure consists of novel concentric trefoil-shaped slot resonators realized using Rogers RT/duroid (R) 5880 laminate, occupying a physical footprint of 13.55 x 13.55 mm(2). Each resonating element is associated with a particular data bit, having a 1:1 resonator-to-bit correspondence. Bit sequences are configured through introducing modifications in the geometric structure either by addition or exclusion of each nested slot resonator. Such changes manifest directly in the electromagnetic signature of the tag as presence or absence of corresponding resonant peaks. The proposed 10-bit tag offers minimized inter-resonator mutual coupling and insensitivity to changes in polarization and incident angles thereby demonstrating orientation independent functionality. Moreover, error-free encoding is achieved through stabilizing the shift in resonant frequencies for a variety of different geometric configurations and orientation of the structure. The tag operates within the license-free ultrawideband ranging from 5.4 to 10.4 GHz, providing spectral bit capacity and bit density of 2 bits/GHz and 5.44 bits/cm(2) respectively
